Who is eligible for Home Care services?

Ask yourself the following questions:
1. Have I recently been diagnosed with a medical condition?
2. Am I recovering from surgery or a hospital stay?
3. Have I experienced a change in health status that requires more care or keeps me at home making it    difficult to receive outpatient medical care?
4. Do I have a medical condition that requires a nurse, physical therapist or speech therapist to work with me    during my recovery at home?

If you answered yes to any of these questions, you may be eligible for home health care services.
However, it is important to remember that your doctor must provide an order for home health care.

Who pays for home health care?

• Medicare**

• Private Insurances
• Private Pay

**Medicare has an established reimbursement criteria that requires the patient to be homebound (i.e. leaving the home is a taxing effort except for infrequent or short absences or trips for medical care) and be in need of at least one of the following skilled service (skilled nursing, physical or speech therapy)

Who can refer patients to Home Health Care?

Patients are typically referred to Home Health Care agency by a physician, hospital discharge planner or a social worker. A nurse or a therapist are sent to clients' homes to conduct an in-home evaluation and confirm a potential client's eligibility for home care services. All referrals must be under the order of a physician.
